Sungai Kedah is one of the most important rivers in Kedah. It flows right through Alor Setar, which is where Sungai Anak Bukit enters it. Sungai Kedah begins at the Alor Setar suburb of Alor Mengkudu, where Sungai Alor Penyengat flows into the larger Sungai Pendang. From there it begins a curling journey westward, passing the city centre of Alor Setar where Sungai Anak Bukit flows into it. The river empties into the Straits of Malacca at Kuala Kedah (the name meaning "mouth of Sungai Kedah").

There are a number of bridges across Sungai Kedah. The last and biggest is Jambatan Tok Pasai at Kuala Kedah. Several other bridges, the majority nameless, cross the river in Alor Setar.
